Job Description We are seeking an experienced LCSW (Licensed Clinical Social Worker) to join the Case Management team at Providence Regional Medical Center Everett. The ideal candidate will have acute-care experience and strong skills in discharge planning, care coordination, biopsychosocial assessment, and case management . Responsibilities Conduct biopsychosocial and patient needs assessments. Provide case management and care coordination. Develop and support individualized plans of care. Assist with discharge planning and transition of care. Coordinate home health services, DME, rehabilitation, and long-term care referrals. Collaborate with physicians, nurses, therapists, and other healthcare professionals. Connect patients with community resources and support services. Document assessments, interventions, and care plans accurately in Epic . Support patients and families with complex psychosocial needs.
Required Qualifications:
Active Washington State clinical social work license required. LCSW/LMSW or applicable Washington social work license. Master's degree in Social Work. Minimum 1 year of experience . Acute care hospital experience required. Previous travel experience required. Epic experience required. Discharge planning and care coordination experience required. Strong communication, assessment, and organizational skills. Preferred Skills Case management experience in Med-Surg, ED, or long-length-of-stay settings. Experience with adult protective services and guardianship. Knowledge of Medicare, Medicaid, and community resources.
AIDA EMR
experience preferred. Experience working with patients requiring complex psychosocial support. LAL01
