Job Description

Job Overview We are seeking a dedicated and compassionate Case Manager/Counselor to join our dynamic behavioral health team. In this vital role, you will provide comprehensive support and counseling services to individuals facing mental health, addiction, and behavioral challenges. Your expertise will help clients navigate their recovery journeys, improve their overall well-being, and achieve their personal goals. Responsibilities Conduct thorough patient assessments, including diagnostic evaluations and intake interviews, to determine individual needs and develop tailored treatment plans. Deliver individual and group counseling sessions utilizing therapeutic modalities such as cognitive behavioral therapy (CBT), behavioral therapy, motivational interviewing, and applied behavior analysis (ABA). Provide crisis intervention and crisis management to ensure client safety during acute episodes or emergencies. Coordinate care with healthcare providers, social workers, families, and community resources to facilitate holistic support for clients. Maintain accurate medical documentation, including progress notes, treatment plans, and medical records in compliance with HIPAA regulations. Utilize diagnostic coding systems such as
ICD-10/ICD-9
for proper documentation of diagnoses and treatment outcomes. Engage in data collection and research activities to monitor client progress and improve service delivery. Qualifications Bachelor's degree in social work, psychology, counseling, or a related field; a master's degree is preferred. Proven experience in case management, mental health counseling, or behavioral health settings. Strong knowledge of psychotherapy techniques, crisis intervention strategies, and addiction counseling methods. Familiarity with medical terminology, medical records management, HIPAA compliance, and CPT coding is highly desirable. Excellent communication skills with the ability to build rapport with diverse populations including children and families. Ability to handle high-pressure situations with professionalism and empathy while managing crisis situations effectively. Certification or licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or equivalent is preferred but not mandatory for all roles.
