Your daily tasks will include:
Responsibilities:

Developing skills within the home environment to foster independence and growth. Some Understanding and implementing of Client Centered Action Plans Providing assistance in accessing community resources and services. Coordinating the delivery of necessary services to meet individual needs. Offering information and resources pertinent to the individual's diagnosis and required supports. Managing scheduling for medical, educational, vocational, and other appointments as needed. Identifying transportation options or services for individuals. Assisting with future planning initiatives. Additionally, you will facilitate activities and routines both at home and within the community to promote stability and permanency for the individuals under your care.

Flexible Schedule Qualifications:

Must have an Associates or Bachelors Degree in a human services or social service field of study plus 1 year of experience in working with adults with developmental, cognitive, or physical disabilities or 3+Years of experience providing Case Management Services for Adults with disabilities. Excellent communication and interpersonal skills are essential. The ability to work independently, demonstrate problem-solving skills, and maintain composure in challenging situations is crucial. Understanding of mental health and developmental disabilities is preferred. Must possess a reliable vehicle, a valid driver's license, and car insurance. Successful completion of a criminal background check is mandatory. Must Speak Spanish fluently (reading, writing, and speaking) Spanish Speaking Required Driver's License Required Ability To Commute To Appointments
