A Mental or Behavioral Health Counselor works with patients to treat mental health problems. Assesses patients and assists with diagnosis, counseling and crisis intervention. Consults with nurses, doctors and psychiatrists. May work with specific populations, such as youth, senior citizens or substance abusers, in group settings, facilities or hospitals.
Inglewood & South Los Angeles Area As a Coordinated Family Support (CFS) Specialist at FMRC Health Group, you'll play a vital role in helping adults with developmental disabilities maintain independence and stability while continuing to live with their families. You'll work directly with clients and families to coordinate essential supports, access community resources, and promote long-term well-being and inclusion. Responsibilities Provide guidance and hands-on support in accessing community and generic resources such as SSI, Medi-Cal, IHSS, and Cal-Fresh , demonstrating strong knowledge of available programs in the Inglewood and South Los Angeles areas. Assess each client's and family's ability to obtain, utilize, and maintain these benefits — including completing applications, renewals, and navigating government systems like Social Security. Coordinate and manage scheduling for medical, educational, vocational, and personal appointments. Identify and arrange transportation options (e.g., Access Services, local mobility programs) to ensure clients can attend appointments and community activities. Support future planning and goal setting to promote independence, long-term stability, and quality of life. Facilitate daily living activities and community participation that foster personal growth, independence, and social engagement. Maintain accurate and timely progress notes, reports, and documentation in accordance with Regional Center and program requirements. Provide coaching in budgeting, communication, and social skill development to help clients thrive at home and in the community. Connect individuals to inclusive social events, volunteer opportunities, and educational or skill-building programs . Collaborate with ILS, SLS, Day Programs, Employment Services, and other agencies to ensure coordinated and comprehensive care. Develop and implement Plans of Action (POA) that outline measurable goals and track client progress.
Qualifications Education & Experience:
Associate's or Bachelor's degree in Human Services, Social Work, Psychology, or a related field plus at least one (1) year of experience working with adults with developmental, cognitive, or physical disabilities; or a minimum of three (3) years of case management experience serving adults with disabilities.
Case Management:
Proven ability to coordinate supports and advocate effectively for clients within complex service systems.
Communication:
Excellent written and verbal communication skills, with the ability to build trust and maintain strong relationships with clients, families, and service partners.
Knowledge:
Solid understanding of public benefit systems and community resources, including SSI, Medi-Cal, IHSS, and Cal-Fresh .
Professional Skills:
Self-motivated, organized, and capable of problem-solving while remaining calm and professional in challenging situations.
Preferred Experience:
Familiarity with mental health services , developmental disability programs, and regional center coordination.
Transportation:
Must have a reliable vehicle , valid driver's license , and current car insurance .
Compliance:
Must successfully complete a criminal background check prior to employment.
Language:
Fluency in Spanish (reading, writing, and speaking) is required.
Flexibility:
Willingness to travel throughout Inglewood and South Los Angeles to meet with clients and families in their homes and communities.
