Overview We are looking for a Bilingual Coordinated Family Support Staff to join our team. This role supports individuals and/or families by identifying needs, researching available resources, and assisting with application for public benefits and services. About Us Inland Family Support Services, Inc is a family support agency based in Temecula, California. We proudly serve individuals and families who are clients of the Inland Regional Center, providing person-centered support and connecting families to essential community resources. The Role The Coordinated Family Support (CFS) program is designed for adults with developmental disabilities who choose to live with their families. This program provides both support and direct assistance to the participants, with the goal of helping adults with disabilities thrive at home. Through CFS services, the consumers and their families are assigned a dedicated CFS staff who assists with their direct needs. This support may include coordinating and scheduling services, identifying and arranging transportation, providing training and guidance for the family members, or helping access essential resources such as SSI, IHSS, Medi-Cal, CalFresh, CalWORKs, or other services the families may require. Approximately 20-30 hours a week depending on caseload. What You'll Do Conduct intake interviews with the consumers and their families to assess their needs and eligibility for services. Develop, implement, and monitor the individualized service plans (IPPs). Provide assistance and training to the consumers and their families in navigating comprehensive services and supports that are tailored to meet the consumer's unique needs to overcome barriers when accessing generic and other resources within reach. Advocate for individuals with intellectual and developmental disabilities to ensure they receive appropriate services. Connect the families to community resources, benefits, housing, transportation, respite care, and other support programs. Assist with scheduling of service delivery including medical and other appointments. Identify and connect the consumers and their families with transportation options or services. Educate individuals and families about available services, rights, and self-advocacy. Maintain accurate case documentation and comply with Inland Regional Center, state, and federal regulations. Ensure services comply with the Developmental Disabilities Services Act and Inland Regional Center policies. Participate in interdisciplinary team meetings and collaborate with other service providers. Assist with crisis intervention and develop plans to address emergency situations. The CFS staff may be assigned additional duties and responsibilities as needed to meet the operational needs of Inland Family Support Services, Inc. During the introductory probation period the new hires will shadow other CFS staff members to obtain more hands-on training. Required Qualifications A.A in Social Services, Social Work, Sociology, and Psychology 3 years working with disabled adults with intellectual disabilities Bilingual required Valid driver's license, reliable transportation, and proof of insurance for mileage reimbursement Must pass background check Will verify references
