POSITION SUMMARY

Seeking master's level, full-time, Health and Wellness Navigator to work with transgender and gender expansive patients at the Transgender Center of the Rockies. The Health and Wellness Navigator will provide comprehensive support to individuals seeking assistance with co-occurring substance use disorders and mental health. This role involves guiding patients through available resources. By offering individual and group therapy, harm reduction and public health education and collaborating to ensure whole person health.

ESSENTIAL JOB FUNCTIONS

• Provide behavioral health evaluations, crisis interventions, and assessments for patients
• Conduct individual and group counseling sessions
• Collaborate with the client to develop, implement, and revise individualized treatment plans and monitor treatment progress.
• Provide case management services to patients as needed or assigned.
• Maintain accurate and timely clinical records consistent with Medicaid, private insurance and the Behavioral Health Administrative Service organizations (BHASO)'s standards in all areas including, but not limited to: admission summaries, assessments, daily charting, weekly clinical staffing notes, and discharge summaries within the agency electronic health record system (EHR).

Adhering to mandatory deadlines.

• Maintain weekly relationships of the patients assigned to you in the active caseload for billable services.
• Complete intakes during scheduled times, as well as providing clinical support during open office hours.
• Provide public health education and risk reduction groups/workshops for patients including testing for

HIV, HIV

intervention and PREP.

• Create and implement harm reduction plans to prevent opioid overdoses and spread of infectious diseases (HIV/HCV/STI).

• Participate in team/department meetings, in-services to learn about other services such as Mind Lift (Ketamine assisted psychotherapy), and supervisory sessions as required.
• Collaborate with all Transgender Center of the Rockies, Element and Bee the Vibe/Harm Reduction and Health and Wellness teams across the agency to ensure interdisciplinary care. Including cross training with HRT gender affirming care.
• Use street outreach strategies to provide harm reduction and intervention to potential and current patients by meeting them in public spaces such as churches, bars, parks, medical facilities and van outreach including fairs etc.
• Professionally represent the agency and program at resource fairs, Denver Pride Fest, and other related programming.
• Complete assessment for gender affirming surgery using the WPATH standards of care; assess the need for on-going treatment or referral to other agencies, as assigned.
• Treat all patients with dignity, respect, equity and equality.
• Perform other duties as assigned.

QUALIFICATIONS

• Knowledge of psychotherapy methods and how it relates to transgender and gender expansive patients.
• Knowledge of community resources.
• Familiarity with the DSM V and clinical assessments
• Knowledge of HIV stigma, sexual health education, and treatment as well as understanding cultural responsiveness.

EDUCATION AND EXPERIENCE

Education:

Master's degree in social work and or Counseling related field. Obtained or working toward clinical licensure and/or Certified Addiction Technician (CAT) and Certified Addiction Specialist (CAS) or Licensed Addiction Counselor (LAC).

• Experience working with behavioral health and substance use patients highly desired.

Strong understanding of Social Justice principles and intersectionality of health disparities in Behavioral Health. Experience working with marginalized communities, particularly transgender and gender expansive folks.

OTHER REQUIREMENTS

• Valid Driver's License and car insurance by date of hire
• Ability to undergo background check due to caring for children, adolescents and families.
• Some travel around the Denver Metro Area
• Some evening and weekend work for events
• This is a full-time in office position (40 hours a week).

Pay:

$62,000.00 - $75,000.00 per year
