Responsible for coordination between DMHAS services and the private sector to facilitate the transition and integration of DMHAS consumers with brain injury into community-based supports and services. The position reports jointly to the

DMHAS ABI

Services Clinical Supervisor for clinical and programmatic supervision around DMHAS referrals. This position reports to the United Services Clinical Manager for administrative supervision and supervision around United Services and consultation clients.

What You Will Do (Day-to-Day): Coordinate services between DMHAS, community providers, and private-sector organizations to support the successful transition and community integration of individuals with acquired brain injuries. Accept referrals from

DMHAS ABI

Services and complete comprehensive biopsychosocial assessments to identify service needs and appropriate community supports. Provide community integration services, including care coordination, waiver application support, benefits assistance, and collaboration with consumers, families, conservators, and service providers. Conduct site visits, home visits, and community-based meetings as needed to support individuals in achieving greater independence and community inclusion. Serve as a liaison among DMHAS, inpatient facilities, advocacy groups, healthcare providers, and community agencies to facilitate discharge planning, service coordination, and continuity of care. Develop and coordinate individualized community service plans, including referrals to behavioral health, rehabilitation, housing, educational, vocational, and support services. Provide education and consultation to others regarding acquired brain injury.

Minimum Requirements:

Current Registered Nurse (RN) Required to maintain and renew licensure as RN in the state of Connecticut or

CT LMSW, LPC-A, LMFT-A

required or

CT LCSW, LPC, LMFT

preferred. Three years experience providing behavioral health care services to individuals and families. Demonstrates cultural competency and a knowledge of mental health and substance abuse disorders.

• ASL and/or Spanish a plus Required Certification/ License (Will be required based on specific program needs): All staff who have their LMSW, LPC-A or LMFT-A and are practicing clinical social work must be licensed, with either a CT LMSW, LPC-A, LMFT-A, LCSW, LPC, or

LMFT. RN

• must be licensed in CT Experience with medical rehabilitation, discharge planning, clinical case management preferred.

Monday

• Friday 8:30 a.

m-5:00 p.m. No weekend or on call expectations.
