09/07/2026 Job Description Job Description About the Role We are seeking a Clinician for an on site position (not fully remote) who is responsible for delivering direct Behavioral Health services to clients. These direct services may include assessment and diagnosis, urgent care, and individual, group, or family therapy. The Clinician will also manage essential case related responsibilities, such as case management, insurance authorization, and disposition planning.Multiple convenient locations including:
428 Columbus Avenue, New Haven, CT (Adult BH) 149 Minor St, New Haven, CT (Adult/BH - New Recovery and Wellness Center Key Responsibilities Provides professional clinical services to individual clients, groups of clients, and/or families in the CS-HHC setting or general community. Maintains client confidentiality at all times and ensures to follow agency, state and HIPAA laws. Formulates bio-psychosocial assessments and diagnosis, treatment plans/reviews and goals according to a site specific time line. Evaluates data relating to the bio-psychosocial consequences of physical and emotional illness, developmental disability and/or substance abuse. Actively monitors client risk factors and takes action to ensure client's safety while following CSHHC policies. Implements standardized evaluations or assessments in conjunction with multidisciplinary team as appropriate. Ensures required documentation is complete, timely and accurate at all times. Obtains and tracks proper insurance authorizations as needed to provide services. Establishes and maintains good working relationships with community agencies. Attends and participates in team conferences (if applicable). Serves as a behavioral health resource to students, teachers, and parents (if applicable). Attends supervision meetings as required. Performs all other duties as required.Required Qualifications Education:
Master's Degree in a Behavioral Health field required.Licensed professionals:
