ESSENTIAL DUTIES AND RESPONSIBILITIES

Complete an integrated assessment of individual client's strengths, needs, abilities, and preferences and revise as necessary for participants in the Assisted Living Facility, other transitional housing placements, and community clients referred for service. Develops an integrated and collaborative individualized care plan with a transition component. Provides the appropriate level of individual therapeutic intervention to support the stabilization and progression of clients. Provides linkage and navigation for health care, supports services and housing stability upon discharge based on individualized needs. Develops and prepares groups to meet client needs, including transition and after care. Arranges for discharge or transfer from services as required by funding and/or program. Coordinates admission of residents to assisted living facilities, nursing homes, residential substance abuse treatment facilities, independent living, and shelters, as appropriate. Provides supports to other disciplines in the residential program including but not limited to nursing, assessment, milieu and Behavioral Health Meets with internal and external interdisciplinary treatment team members to coordinate client services and documents outcomes of meetings, including hospitals, health department and medical providers. Submits all documentation in an accurate and timely manner, including electronic records Achieves and maintains program established productivity levels. Promotes program refinement, understands client/public need, and effectively participates in all assigned meetings. Engages clients in a strength-based supportive manner providing re-directions and guidance as necessary. Engages, counsels, manages, motivates, and effectively works with high-risk, vulnerable population, including chemically dependent persons and persons in crisis.

EDUCATION AND WORK EXPERIENCE

Associate degree in a social service or health related field Or Licensed Practical Nurse with 3 years case management/social services experience

HIV/AIDS

knowledge preferred, Experience with individuals impacted by Homelessness preferred

CERTIFICATES AND LICENSES

HIV 500 Valid Florida Drivers License and ongoing proof of insurance required. HOPWA training modules
