Compensation:
$27.00 per hour + Shift Differential. Position Summary Southern Winds Hospital is seeking a compassionate and dedicated master-level Mental Health Counselor or Clinical Social Worker to provide behavioral health services to our inpatient population. The Case Manager is responsible for conducting psychosocial assessments, coordinating treatment and discharge planning, facilitating therapeutic interventions, and collaborating with the interdisciplinary team to ensure high-quality patient care. This position is ideal for clinicians who are committed to improving the lives of individuals experiencing mental health challenges while working in a collaborative and supportive hospital environment. Essential Duties and Responsibilities Complete comprehensive intake assessments, psychosocial evaluations, and individualized treatment plans in accordance with hospital policies and regulatory standards. Provide individual, family, and group counseling to support patient treatment and recovery. Develop and coordinate discharge plans to ensure continuity of care and successful transition to community resources. Collaborate with psychiatrists, physicians, nurses, and other members of the interdisciplinary treatment team. Monitor patient progress and participate in weekly treatment plan reviews. Maintain timely, accurate, and complete clinical documentation. Assess psychosocial factors that impact patient treatment and recovery. Demonstrate knowledge of Florida mental health laws, including the Baker Act, and initiate involuntary examination procedures when appropriate. Assist with petitions for involuntary admissions as required. Monitor treatment compliance and identify psychosocial or medication-related concerns, particularly among geriatric patients. Implement therapeutic interventions appropriate to each patient's clinical needs. Serve as a clinical resource to the treatment team regarding psychosocial issues affecting patient care. Maintain patient confidentiality and comply with all hospital policies, state regulations, and accreditation standards. QualificationsRequired Master's degree in Mental Health Counselor or Clinical Social Worker Working knowledge of Florida mental health laws, including the Baker Act. Strong clinical assessment, crisis intervention, documentation, and communication skills. Ability to work independently and as part of a multidisciplinary treatment team. Preferred Previous inpatient psychiatric or behavioral health experience. Experience working with geriatric or older adult populations. Knowledge of discharge planning and community behavioral health resources. Physical Requirements Ability to walk throughout the hospital and respond promptly to patient care needs. Ability to sit, stand, bend, and lift up to 25 pounds as needed. Ability to respond appropriately in crisis or emergencies. Southern Winds Hospital is an Equal Opportunity Employer.
