Job Description

Community-based CORE service provider seeking Fully Licensed Clinicians (LCSW, LMFT, LPC), Associate Licensed Clinicians (APC, AMFT, MSW), and Master's level clinicians to provide community-based services in the home and other community settings to individuals that are diagnosed with mental health, substance abuse, and/or co-occurring disorders. This job is community-based and one must live within the Metro Atlanta Area. Telehealth service provision is limited. Responsibilities include (but are not limited to):
  • Provide therapeutic interventions for clients of all ages who meet criteria for services and their families through individual and family counseling/therapy.
  • Maintain a caseload of clients with mental health, substance abuse, and/or co-occurring disorders.
  • Collaborate with assigned staff and other identified supports to implement individualize recovery plan.
  • Complete documentation including but not limited to services rendered and discharge summaries in a timely manner.
  • Maintain detailed case files and other documentation in 100% compliance with standards.
  • Support and maintain an environment of emotional and physical safety for clients, staff and other stakeholders at all times.
  • Ensure confidentiality and rights of clients served are maintained at all times.
  • Attend necessary meetings as required.
  • Maintain certifications/trainings and provide information to HR in a timely manner.
