3005094468-2 Apply for this job now Job Description Health Connect America seeks an Independent Contractor for Family Support Services to provide in home and community based support to children, adolescents, and families. In this flexible role, you will deliver skill building, parenting support, crisis intervention, and resource linkage under a client centered, trauma informed approach. Collaborate with therapists, case managers, and community partners to implement service plans, document progress, and help families build stability, safety, and resilience. Ideal for mission driven professionals experienced in behavioral health, social services, or family support who value autonomy, teamwork, and community impact. Responsibilities Provide in-home and community-based family support services to children, adolescents, and families. Implement goal-focused service plans to build parenting skills, family stability, and resilience. Offer crisis intervention and de-escalation, following trauma-informed and client-centered practices. Coordinate with therapists, case managers, and community partners to ensure integrated care. Connect families with community resources and supports for housing, education, healthcare, and basic needs. Maintain timely, accurate documentation of visits, progress notes, and outcomes. Monitor family safety and risk factors, reporting concerns in alignment with policy and regulations. Participate in team meetings, supervision, and trainings to enhance service quality and skills.
