In this rewarding role, you'll work directly with adults living with severe and persistent mental illness and/or substance use disorders, helping them build the practical life skills they need to thrive in their homes and communities. What You'll Do As a Skills Trainer, you'll provide one-on-one, community-based support that empowers clients to live healthier, more fulfilling lives.
Responsibilities include:
Teach and reinforce daily living, self-care, communication, and social skills using evidence-based practices. Develop individualized skill-building plans based on each client's unique goals and needs. Help clients gain confidence and independence in everyday activities. Educate clients and their families about mental health, substance use, recovery, and available community resources. Connect clients with community resources and encourage participation in meaningful community activities. Collaborate with families, caregivers, treatment providers, community agencies, DCS, court systems, and other partners to support client success. Maintain timely, accurate documentation in the electronic medical record (EMR). Participate as an active member of each client's treatment team while maintaining the highest standards of ethics, confidentiality, and professionalism. High School Diploma or GED 2 years of experience in skills training, social services, or a related field. Relevant certifications in skills training or related areas are a plus (e.g., Certified Rehabilitation Counselor (CRC), Certified Life Skills Coach). Valid driver's license, insurance and reliable transportation to visit clients in the community.
