Job Description

Position Summary:
  • Provides a wide range of case management, educational, solution focused and supportive services to clients with persistent mental health and substance abuse issues.
  • Identifies effective strategies and emotional/behavioral interventions designed to help clients minimize their response to triggers; restore social skills needed to promote effective functioning in work, school, and home.
  • Advocates for clients in the community and works with providers, family members, employers and landlords to assist client in establishing a support network and enhance personal and professional relationships.
Principle Duties/Responsibilities:
1. Works with clients to develop appropriate activities for daily living, including financial management and utilization of community resources. 2. With client's participation, implements individual treatment plans to support optimal functioning. 3. Participates in treatment planning meetings as required. 4. Ensures appropriate community referrals and follow up care. 5. Maintains accurate, complete and timely documentation of all services as required. 6. Assist with crisis stabilization and behavioral de-escalation as needed. 7. Participates in agency's quality management and utilization review. 8. Maintains knowledge of current trends/developments in the field by reviewing books, literature, and attending continuing education workshops. 9. 1. Education
  • High School Diploma or equivalent degree, training in either mental health or substance use disorder competencies. 2. Experience
  • Basic mental health or substance use disorder and recovery skills 3.
