Substance Use Disorder Counselor (CAC-AD / CSC-AD / LGADC / LCADC) -- Job Summary We are hiring an Intake & Admissions Counselor to own the front door of the program: first contact, screening, comprehensive assessment, ASAM level-of-care determination, and admission. This is a clinical position, not a scheduling one — you will be doing assessments and making level-of-care recommendations. How well this seat runs determines whether a person who calls on Monday is in treatment by Thursday or gone. What you will do Take initial inquiry calls and referrals from Mid Shore Behavioral Health, the Talbot County Health Department, hospitals, courts, and self-referrals, and complete telephone screening the same business day where possible. Verify insurance eligibility and benefits at inquiry and identify coverage barriers before the assessment appointment, referring benefit problems to the appropriate staff member. Complete the comprehensive biopsychosocial assessment and ASAM level-of-care determination at a scope consistent with your credential, with the Clinical Director holding final authority on level of care. Prepare and submit initial authorization requests before service initiation and record the authorization number, span, units, and approved setting in the client record and the authorization tracker. Complete the admission packet with the client: consents, client rights and responsibilities, release of information, program rules, grievance procedure, and MOUD non-discrimination notice. Develop the initial treatment plan with the client within 7 working days of the comprehensive assessment and hand off to the assigned counselor with a documented warm transfer. Conduct risk assessment at intake for suicidal ideation, self-harm, overdose risk, and homicidal ideation, and escalate to the Clinical Director immediately where indicated. Deliver overdose recognition and naloxone training at admission and document completion. Coordinate MAT starts with the contracted telehealth prescriber so that a client entering with opioid use disorder has a prescriber appointment scheduled at admission, not after. Refer out or divert to a higher level of care where the assessment indicates the client's needs exceed Level 2.1, documenting the referral and following through to a confirmed disposition. Track inquiry-to-admission conversion and time to first appointment, and report both to the Clinical Director monthly as a quality indicator. Carry a small ongoing caseload or group assignment as census allows, per the written assignment on file. Experience Experience in addiction counseling or mental health and behavioral care is preferred. Prior experience working with individuals who have co-occurring disorders is highly desirable. Familiarity with client assessment techniques used in therapy settings. Ability to work effectively with clients from diverse cultural backgrounds. Knowledge of EMR/EHR systems for documentation purposes is advantageous. Candidates should possess a Maryland counselor credential at the
