Field-Based (Local Travel Required) / Remote Documentation Position Summary TrueHaven Hospice is a growing healthcare startup dedicated to bringing expert medical care and gentle human presence to families navigating life's final chapters. We are seeking a flexible, autonomous Bereavement Counselor on a need-based (PRN) basis.

In this role, you will scale your hours alongside our patient census, conducting risk assessments and providing short-term grief counseling to families and caregivers. This position is ideal for a self-starting clinician who thrives in a nimble startup environment and values clinical independence. Key Responsibilities Flexible Case Management & Counseling Conduct phone-based or in-home bereavement risk assessments for families within mandated regulatory timelines following a patient's passing. Provide high-quality, short-term individual or family grief counseling on an as-needed basis. Formulate efficient Bereavement Care Plans tailored to low, moderate, or high-risk grief indicators. Provide crisis intervention resources or external clinical referrals for families experiencing complicated or prolonged grief. Startup Agility & Compliance Complete all clinical visit notes within our Electronic Medical Record (EMR) system within 24 hours of contact to ensure total compliance and audit readiness. Maintain absolute compliance with Medicare Conditions of Participation (CoPs) regarding the 13-month post-loss bereavement tracking requirement. Collaborate asynchronously or attend virtual Interdisciplinary Group (IDG) meetings to communicate family risk levels to the core care team. Qualifications & Requirements Education & Licensure Master's Degree in Social Work (MSW), Mental Health Counseling, or Psychology from an accredited institution. Active, unrestricted state clinical license required (e.g., LMSW, LCSW, LPC, LMFT). Experience & Soft Skills Minimum of 1 year of experience in hospice, palliative care, oncology, or dedicated grief therapy. High comfort level with technology, remote communication tools, and mobile EMR platform documentation. Exceptional time-management skills and the ability to operate independently without daily micro-management. Entrepreneurial mindset—comfortable with the evolving workflows of an early-stage healthcare startup. Additional Requirements Valid driver's license, reliable vehicle, and active auto insurance for required local family visits. Ability to pass standard healthcare background checks and drug screenings. Compensation Competitive hourly / per-visit rate commensurate with experience and licensure level.

$33.34 - $40.15 per hour
