Responsibilities Complete intake and bio-psychosocial assessments, crisis plans, and required documentation. Assess mental health, behavioral, and co-occurring substance use needs. Facilitate Person-Centered Planning and develop appropriate treatment recommendations. Provide crisis intervention and referrals as needed. Utilize DSM criteria to support clinical diagnoses. Coordinate members with appropriate clinical staff and ensure follow-up appointments are scheduled. Maintain timely and accurate documentation in the Electronic Medical Record (EMR). Maintain compliance with agency, credentialing, and training requirements. Qualifications Master's degree in Social Work, Counseling, Psychology, or related behavioral health field .
