This position is responsible for providing Intensive Care Coordination Wraparound (ICCW) for children/youth with serious emotional disturbances and intellectual developmental disabilities ages 0-21 years, involved with multiple community systems. This is a strength-based approach to identifying family resources, supports, and needs to provide highly coordinated care among all treatment providers and natural support systems. A family team works together to assist the family in setting goals and accomplishing tasks with the ICCW Specialist facilitating the process. Services are provided in a community setting. Additionally, manages enrollment and disenrollments in the SEDW. Promotes mental health recovery, wellness, and full participation within the community for persons-served in a manner where practice is culturally competent, trauma informed, co-occurring capable, person/family centered and integrated with other health care provide

Student Loan Repayment :
MCN is an approved National Health Service site (NHSC, a loan repayment program); in addition to Public Service Loan Forgiveness (PSLF), and MDHHS Behavioral Health Loan Repayment Program (BHLRP). Tuition reimbursement up to $5,250 annually
Employment Qualifications:
Education:
Possession of a bachelor's degree in social work or human services related degree.
Experience:
One year of experience in a mental health related field in the examination, evaluation, and treatment of minors and their families required.
Other Requirements:
If a bachelor-degreed Social Worker, must be licensed in the State of Michigan. Must be Children's Mental Health Professional (CMHP) or must be supervised by an individual with this credential. Possession of a valid State of Michigan driver's license and ability to obtain and maintain a chauffeur's license within 30 days of hire. Local travel is an essential function of this role.
