The Outpatient Counselor provides direct patient care and case management services as part of Project Turnabout's multidisciplinary treatment team. This position manages an assigned caseload and works closely with patients to assess needs, develop individualized treatment plans, monitor progress, and coordinate continuing care.

Key Responsibilities:

Manage an assigned patient caseload and coordinate individualized treatment services. Conduct assessments and assist patients in developing and implementing treatment plans. Facilitate individual and group counseling sessions, family programming, and addiction/recovery education. Collaborate with multidisciplinary team members, referral sources, families, and other professionals involved in patient care. Complete timely and accurate clinical documentation, including assessments, progress notes, treatment plans, continuing care plans, and discharge summaries. Lead patient staffing discussions and contribute to treatment planning for patients across the program. Utilize ASAM criteria and maintain compliance with Minnesota 245G requirements, patient privacy standards, and organizational policies. Participate in staff meetings, training, continuing education, and professional development.

Qualifications:

Must meet Minnesota licensure requirements or be eligible for temporary licensure through the Minnesota Board of Behavioral Health and Therapy. Associate degree or higher in Alcohol and Drug Counseling or a related field, as required for licensure. Strong written, verbal, interpersonal, and computer skills. Ability to work collaboratively within a multidisciplinary treatment team. Must pass a Minnesota Department of Human Services background study and meet all applicable regulatory requirements.
