A Mental or Behavioral Health Counselor works with patients to treat mental health problems. Assesses patients and assists with diagnosis, counseling and crisis intervention. Consults with nurses, doctors and psychiatrists. May work with specific populations, such as youth, senior citizens or substance abusers, in group settings, facilities or hospitals.

Job Summary:
We are seeking a licensed Clinician to provide therapeutic services including assessments, individual, group, and family counseling. You'll be part of a multidisciplinary team focused on delivering high-quality mental health services.
Key Responsibilities:
Conduct Comprehensive Assessments:
Complete assessments using standardized and evidence-based tools to evaluate client needs and determine appropriate diagnosis and level of care.
Develop Individualized Treatment Plans:
Collaborate with clients to create treatment goals that are specific, measurable, attainable, relevant, and time-bound (SMART), addressing their unique needs and strengths.
Deliver Individual and Group Therapy:
Provide evidence-based counseling services including cognitive-behavioral therapy (CBT), solution-focused therapy, and other modalities to support client progress.
Facilitate Family Therapy Sessions:
Engage families in treatment planning and family sessions to support systemic healing, communication, and behavioral change.
Track Progress and Adjust Plans:
Continuously monitor client engagement and outcomes, updating treatment plans as necessary to reflect new goals or evolving clinical needs.
Develop Discharge and Transition Plans:
Prepare clients for discharge with appropriate referrals, follow-up resources, and continuity of care planning. Coordinate with
Community Partners:
Maintain active communication with referral sources such as schools, courts, DCP&P, healthcare providers, and other community agencies. Participate in
Interdisciplinary Team Meetings:
Collaborate with colleagues including case managers, psychiatrists, nurses, and peer support staff to ensure cohesive and client-centered care.
Ensure Accurate Clinical Documentation:
Maintain timely, accurate, and complete progress notes, treatment plans, and billing documentation within the Electronic Health Record (EHR) system. Contribute to a
