Job Description

Position Summary:
Rutgers, The State University of New Jersey, is seeking a Habilitation Counselor for the Edison Outpatient Services within Rutgers University Behavioral Health Care. The purpose of the Habilitation Counselor position is to provide site and community-based case management and supported employment services to individuals receiving treatment services through UBHC's Outpatient Coordinated Specialty Care ( CSC ) services. Among the key duties of this position are the following: Links individuals served to needed services including benefits, medical, dental, mental health, co-occurring services, self-help, employment, legal and housing. Utilizes motivational interviewing skills to establish a working relationship with individuals served. Maintains professional, effective, open communication with all members of the program's interdisciplinary team. Writes clearly and effectively as reflected in clinical documentation that is concise and well organized. Attends and participates in in-service meetings and other designated training events that will enhance skills, as documented by continuing education reports. Demonstrates knowledge of University Behavioral Health Care's vision, mission, and values as these relate to daily activities, decisions, and quality improvement priorities.
Minimum Education and Experience:
Bachelor's Degree in the mental health field or other appropriate human services related field plus one (1) year of relevant mental health experience.

Ability to speak clearly and write proficiently. Hearing is essential. The ability to focus on the task at hand despite numerous interruptions. Must be able to sit for extended periods of time and document into an electronic medical record. The employee spends much time driving, walking, standing, bending and talking with consumers in the community located throughout the County. Transport of consumers in state vehicles. The employee can expect to lift up to 30 pounds. The work environment includes both office based services and community outreach. Individual wellness checks and crisis intervention required. Subject to unpredictable situations, patient crises, to include behavioral, medical and psychiatric. Occasionally exposed to patients exhibiting assaultive behaviors. Exposure to blood borne pathogens that requires use of personal protective equipment.
