Job Description

Overview We are seeking a dedicated and compassionate Intensive In-Community Licensed Clinical Social Worker (LCSW), Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), or Licensed Alcohol and Drug Counselor (LAC) to provide comprehensive mental health and behavioral support within community settings. This role offers an exciting opportunity to work directly with children, adolescents, and their families in home-based environments, delivering vital therapeutic services that promote growth, resilience, and well-being. The ideal candidate will possess a strong background in behavioral health, crisis intervention, and family therapy, with a passion for making a meaningful difference in clients' lives through evidence-based practices. Must be willing to treat the individual in their home or community. Duties Conduct thorough client assessments to identify mental health needs, developmental challenges, and behavioral concerns using standardized diagnostic tools and clinical judgment. Deliver individual, group, and family counseling sessions utilizing modalities such as cognitive behavioral therapy (CBT), and trauma-informed care to address diverse client needs. Implement home-based behavioral interventions and community care plans tailored to children with disabilities, neurodevelopmental disorders, or behavioral health conditions. Engage in crisis intervention to de-escalate situations effectively while ensuring client safety and stability. Collaborate closely with families, educators, medical providers, and case managers to develop comprehensive treatment plans that support client progress across multiple environments. Maintain accurate clinical documentation adhering to HIPAA regulations and medical documentation standards within electronic health records (EHR) systems. Participate in ongoing professional development to stay current with best practices in social work, addiction counseling, child & youth therapy, and cultural competency. Skills Proficiency in motivational interviewing techniques to foster client engagement and facilitate behavior change. Extensive experience working with individuals with developmental disabilities and neurodevelopmental disorders such as autism spectrum disorder. Strong background in addiction counseling, trauma-informed care including PTSD management, and crisis management strategies. Skilled in conflict management, behavioral management techniques, and family therapy approaches to strengthen client support systems. Knowledge of medical terminology, healthcare workflows, and electronic health records management for accurate case documentation. Ability to conduct diagnostic evaluations and implement evidence-based interventions like applied behavior analysis (ABA) and cognitive behavioral therapy (CBT). Cultural competency skills necessary for working effectively with diverse populations across various community settings.
