About the Role We are seeking qualified licensed behavioral health professionals to provide CORE Community Psychiatric Support and Treatment (CPST) services to adults enrolled in Medicaid Health and Recovery Plans (HARP).

CORE CPST

is a goal-directed, recovery-oriented service designed to improve functioning, promote independence, support recovery, and reduce barriers related to serious mental illness (SMI) and co-occurring conditions. This is an excellent opportunity for clinicians who enjoy community-based behavioral health work , building meaningful relationships with individuals, and supporting clients in achieving their treatment and recovery goals.

CORE CPST

services to adults with serious mental illness (SMI) Conduct comprehensive behavioral health assessments Develop and implement Individual Service Plans (ISPs) Provide goal-directed, recovery-oriented behavioral health interventions Utilize evidence-informed approaches, including: Motivational Interviewing (MI) CBT-informed strategies Harm reduction approaches Trauma-informed practices Support symptom stabilization and recovery Assist with crisis prevention and safety planning Promote community integration and independence Support independent living and skill development Coordinate care with Health Home Care Managers, psychiatric providers, primary care providers, and community-based service providers Maintain accurate, timely, and Medicaid-compliant documentation Participate in clinical supervision and interdisciplinary team meetings Qualifications Active New York State license listed above Experience working with adults with serious mental illness (SMI) preferred Experience with co-occurring mental health and substance use disorders preferred Knowledge of Medicaid-funded behavioral health services Strong clinical documentation and treatment planning skills Strong crisis intervention and risk assessment skills Excellent communication and engagement skills Ability to work independently in community-based settings Ability to maintain professional boundaries Comfortable providing services in members' homes and other community settings Ability to travel locally as needed Willingness to work flexible hours, including evenings or weekends as needed Training Requirements Providers will be required to complete applicable CORE CPST and program-required training, including: CORE-required CPST training through the CPI Learning Community Trauma-Informed Care Mandated Reporter Training Safety in the Community / Personal Safety Suicide Prevention Cultural Humility Recovery-Oriented Practices Required continuing education Bilingual and multilingual licensed professionals are strongly encouraged to apply.
