- Community Missions has three (3) Senior Residence Counselor positions throughout the agency.
- Conduct assessments of residents' strengths and needs.
- Develop and implement individualized care plans that address resident needs and goals
- Monitor resident progress through regular follow-ups and adjust care plans as needed
- Advocate for residents to ensure they receive appropriate services and support.
- Maintain accurate and up-to-date case files and documentation in compliance with regulatory requirements and policies.
- Assist residents with daily living activities, such as meal preparation, medication management, and cleaning.
- Provide crisis intervention and support to residents in emergency situations
- Ensure compliance with all relevant policies, procedures, and regulations
- On-call responsibilities in rotation with a team
- Hours include evening, weekend, and holidays. Qualifications
- Bachelors degree (preferred) in psychology, social work, counseling, or a related field plus 1 year professional experience in the human services field
- Valid New York State Driver's License with clean driving record

Full-time Pay:
$41,729.00- $51,563.
