Job Description

Senior Residence Counselor Location

  • Full Time Job Summary
  • Community Missions has three (3) Senior Residence Counselor positions throughout the agency.
The positions are responsible for the implementation of trauma informed and person-centered individualized service plans. In addition, they supervise daily activities and maintain a safe and supportive atmosphere for the residents. Senior Residence Counselors work closely with residents, their families, and a network of service providers to ensure the residents receive the necessary support to achieve their personal goals. Key Responsibilities
  • Conduct assessments of residents' strengths and needs.
  • Develop and implement individualized care plans that address resident needs and goals
  • Monitor resident progress through regular follow-ups and adjust care plans as needed
  • Advocate for residents to ensure they receive appropriate services and support.
  • Maintain accurate and up-to-date case files and documentation in compliance with regulatory requirements and policies.
  • Assist residents with daily living activities, such as meal preparation, medication management, and cleaning.
  • Provide crisis intervention and support to residents in emergency situations
  • Ensure compliance with all relevant policies, procedures, and regulations
  • On-call responsibilities in rotation with a team
  • Hours include evening, weekend, and holidays. Qualifications
  • Bachelors degree (preferred) in psychology, social work, counseling, or a related field plus 1 year professional experience in the human services field
  • Valid New York State Driver's License with clean driving record
