Job Description
Under immediate to general supervision, as part of a Wraparound Team, provides targeted support services to provide a system of care that is child-centered with the needs of the child and family dictating the types and mix of services provided to assist in preventing an out-of-home and out-of-school placement. This position's general and immediate supervision is to be determined. The Family Support Provider will ensure that service provision documentation are conducted to the highest standards of fidelity of the Wraparound Care Model.
