Overview:
Sentara Health Plans is hiring a Care Coordinator for LTSS Waiver in Charlottesville, VA and the surrounding areas!
Status:
Full-time, permanent position (40 hours) Standard working hours: 8am to 5pm
EST, M-F
The position requires conducting both in-person and telephonic health risk assessments for Medicaid members receiving LTSS waiver services in Charlottesville and the surrounding area, including face-to-face member visits up to 3-4 days per week.
Location:
Candidates must reside in Charlottesville or within a 30-60 minute drive east or southeast of Charlottesville, including areas such as Keswick, Scottsville, Palmyra, and surrounding communities.
Job responsibilities:
Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses. Education Must possess a degree in Health & Human Services or one of the following related
