Job Description
Position Summary The Intensive In-Home Qualified Professional (QP)/Case Manager is responsible for assisting the Team Lead with the coordination of services across all levels of care. This individual will directly participate in and assist the Team Lead with clinical documentation, facilitation of treatment team meetings, psychoeducation, and skill development. The QP must have at least one year of experience with children or adolescent mental health consumers and is responsible for ensuring that consumers receive the appropriate treatment and support to help them manage their condition and improve their quality of life. Entry Requirements Currently licensed by the appropriate North Carolina licensure board as a Licensed Clinical Addiction Specialist, Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, Licensed Professional Counselor, Psychiatrist, Licensed Psychologist or a Licensed Psychological Associate, except a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience in mh/dd/sa with the population served OR Master's degree in a human service field plus one year of full-time, post-graduate degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional (with a master's degree in a human service field) who has one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR Bachelor's degree in a human service field plus two years of full-time, post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional (with a bachelor's degree in a human service field) who has two years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR Bachelor's degree in a field other than human services and has four years of full-time, post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional (with a bachelor's degree in a non-human services field) who has four years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling Roles and Responsibilities Provide direct and indirect therapeutic interventions in a variety of settings, including, but not limited to, home, school and community. Interact with parents/stakeholders as necessary. Provide psychoeducation (information about consumer's diagnosis and effective interventions) to parent/caregiver, school personnel, other agencies involved with consumer. Interact with school personnel regarding consumer's schedule/behavior/etc. Document consumer's progress notes in electronic health record system (EHR). Plan each consumer's monthly treatment team meeting. This includes formally inviting all stakeholders and team members. Meet with other stakeholders involved with consumer (DJJP, school, DSS, other service providers). Monitor the provision of other services to determine if these services are effective. Assess consumer's need for additional services (i.e., determining if consumer needs medical/dental/or other psychiatric services). Schedule consumers for psychiatric evaluation. Plan contact with parent regularly to provide updates on treatment plans, activities, case management, psychoeducation. Assist with coordinating annual Diagnosis Assessment and Psychiatric Evaluations. Complete other relevant duties as assigned by leadership. Part-time/Full-time position that reports to
