Overview Revitalize Health Center LLC is expanding our Behavioral Health services and seeking a compassionate and motivated Florida Licensed Mental Health Counselor (LMHC) to join our growing multidisciplinary healthcare team. This is a part-time position with the opportunity to grow as our behavioral health patient population expands . Revitalize provides an established clinical and administrative infrastructure, allowing the counselor to focus primarily on providing high-quality psychotherapy and behavioral health services. Our patient population primarily consists of adults and older adults , including individuals managing depression, anxiety, adjustment difficulties, chronic medical conditions, cognitive changes, grief, and other behavioral health needs. Responsibilities Provide individual psychotherapy and behavioral health counseling to adult and older-adult patients. Conduct comprehensive mental health assessments and diagnostic evaluations. Develop individualized treatment plans based on each patient's clinical needs and goals. Utilize evidence-based therapeutic approaches, including Cognitive Behavioral Therapy (CBT), supportive psychotherapy, behavioral interventions, grief counseling, and other clinically appropriate modalities . Monitor patient progress and update treatment plans as clinically indicated. Identify changes in mental health status and appropriately address safety concerns or the need for a higher level of care. Provide psychotherapy through telehealth and, as the program develops, potentially in-person services. Maintain timely and accurate clinical documentation, including assessments, treatment plans, progress notes, and discharge documentation. Maintain documentation consistent with professional, HIPAA, payer, and regulatory requirements. Collaborate with psychiatric, primary care, and other healthcare providers to support coordinated patient care. Provide education and support to patients and caregivers when clinically appropriate. Participate in care coordination for patients with both behavioral health and chronic medical conditions. Qualifications Active and unrestricted Florida Licensed Mental Health Counselor (LMHC) license - REQUIRED Active individual NPI Master's degree in Mental Health Counseling or a related qualifying behavioral health field. Experience providing individual psychotherapy. Experience working with adults and/or older adults preferred. Experience with Medicare and/or Medicaid populations is a plus. Familiarity with behavioral health diagnosis and ICD-10 documentation . Experience with EMR/EHR clinical documentation. Telehealth experience preferred. Strong clinical judgment, communication, and documentation skills. Ability to work independently while collaborating with a multidisciplinary healthcare team. Schedule Part-time to start , with the opportunity for additional hours as our Behavioral Health program and patient census continue to grow.
