Tailored Care Manager
QUALIFICATIONS
Must be at least a QP/CSAC or an Associate or Fully Licensure in a behavioral health discipline (LCAS-A/ LCAS, LCMHC, LPA, LCSW-A/ LCSW.) Experience providing services to people with co-occurring behavioral health and substance use disorders. Must have experience with MH/SA population.•Major Duties and Responsibilities Arrange, Coordinate and Link Services for Members (adults and children) Ø Collaborate with other professionals to evaluate clients mental health and substance abuse condition and to assess client needs Ø Determine nature and extent of disorder and formulates treatment program with other practitioners on the team. Ø Participate in Treatment team meetings and develop plans Ø Maintain case records correspondence, supervision notes and files in accordance with procedures Ø QP will be available for urgent and emergency consultation after office hours Ø Position must have access to computer, fax and telephone after office hours
Agency Responsibilities :
These include attendance at professional meetings (include peer and clinical supervision, supervision meetings) and trainings to keep abreast of agency regulations and procedures, increase knowledge and skills in clinical areas, and provide needed agency services, documentation of clinical work with clients and keeping of medical records per agency and Medicaid standards. The Agency's requirements include annual Blood Borne Pathogens training for all staff and NCI training. Clinical Requirements Ø Individual counseling and support Ø Group counseling and support Ø Family counseling, training or support Ø Life skills training and development Ø Crisis contingency planning Ø Treatment support activities that have been adapted or specifically designed for persons with physical disabilities, or persons with co-occurring disorders of mental illness and substance abuse dependence or mental retardation developmental disability and substance abuse dependence (if applicable) Knowledge, Skills, Abilities Ø Knowledge of substance abuse and mental health best practices Ø Knowledge of theoretical applications to substance abuse counseling Ø Documentation•Knowledge of NC Medicaid documentation requirements Ø Ability to handle crisis emergency situations and to utilize conflict resolution skills Ø Knowledge of word processing and data management Ø In depth knowledge about community mental health and related health systems, including but not limited to: knowledge of CARF guidelines and policies, confidentiality, professional standards for treatment, applicable code(s) of ethics, and the requirements of the presiding local management entity Ø Proficiency with utilizing basic equipment, including personal computer, fax, copier, printer/scanner, phone and other materials used in the community based therapeutic practices (This accuracy requires visual/perceptual, verbal and written communication skills)
