Job Description

Mental Health Professional Hart to Heart Therapy, LLC is a unique mental health provider that is looking for a Mental Health Clinician to join our team. We are a rapidly growing company and are looking for the right person to grow with us.
Job Description:
There are three positions open currently. We are seeking professionals in the following areas: LPC or LPCA, LMFT or LMFTA, LMSA or LISW, LACA or LAC. The Mental Health Professional (LPC/LPCA, LMFT/LMFTA, LMSW/LISW, LACA/LAC) will be responsible for providing quality mental health services to meet the individual needs of the patient. All counselors and therapists will be responsible for documentation of therapeutic services. The Licensed Professional Counselor Associate (LPCA), Licensed Marriage and Family Therapist Associate (LMFTA), and/or Licensed Addiction Counselor Associate (LACA) will be provided with supervision (individual and group) and office space. All counselors with practice documentation of therapeutic activities, monitoring progress, and treatment for clients. This position will also assist with the administration of the treatment program, as well as coordinate intake, discharge, and referrals for patients.
Responsibilities:
  • Complete intake paperwork accurately and efficiently in accordance with local regulations.
  • Assist with treatment planning by coordinating intake forms, discharge paperwork, progress notes, and referrals for BOTH adults, teenagers, and children where needed.
  • Provide documentation of therapy sessions by completing weekly reports on sessions notes.
  • Assist with maintaining patient records by completing weekly reports on records, completing charting as needed, and ensuring accurate documentation of services provided.
  • Maintain positive patient relations by responding to patient inquiries in a timely manner and maintaining positive communication throughout the course of care.
  • Maintain confidentiality of all patient information including but not limited to medical history, medications, and diet information.

$40.00 - $60.00 per hour
