Full Job Description Job Summary:
Help older adults thrive by providing compassionate, person-centered behavioral health care that supports their emotional well-being and quality of life. As a Behavioral Health Specialist (LICSW-Therapist), you'll conduct comprehensive behavioral health assessments, develop individualized treatment plans, and provide counseling and therapeutic interventions to PACE-RI participants. Working as an integral member of the Interdisciplinary Team (IDT), you'll help ensure each participant's behavioral health needs are thoughtfully integrated into their overall plan of care. This rewarding role is ideal for a licensed behavioral health professional who is passionate about serving older adults and values a collaborative approach to care. You'll partner with participants, families, caregivers, and fellow healthcare professionals to address mental health needs, provide crisis intervention and education, connect individuals with community resources, and promote positive outcomes.
