Job Description

This position requires working with the population served. Assess and treat individuals with mental, emotional, or substance abuse problems, including abuse of alcohol, tobacco, and/or other drugs. Activities may include individual and group therapy, crisis intervention, case management, client advocacy, prevention, and education.
ESSENTIAL JOB FUNCTIONS
Conducts psychosocial assessments related to mental health diagnoses to provide input to Provides ongoing individual, family and group counseling Counsel clients in individual or group sessions to assist them in dealing with substance abuse, mental or physical illness, poverty, unemployment, or physical abuse. Counsel clients or patients with substance abuse issues. Counsel clients or patients regarding personal issues. Consults with medical provider for input and/or approval at each phase, related to implementation of individual treatment Assists patients and their families in understanding and conforming to prescribed treatment(s). Links patients and their families to required resources and services: provides basic casework services with follow-up as Assess, plan, implement and follow up with appropriate intervention for each referred Collects, organizes, analyzes data, inpatient records, progress reports, discharge summaries, treatment plans; makes recommendations for follow up care. Provides social work services according to established program protocols. Maintains confidential counseling files and charts to quality management for review when Keeps records of social work activities in accordance with professional and program standards with attention to the needs for data in relation to quality management, program planning and reporting. Prepares monthly progress reports and submits to the Must hold all patient Protected Health Information and patient personal information in Obtain continuing education to remain up to date in field Position will require the ability to travel to our Easley office as necessary to see patients.
MINIMUM JOB REQUIREMENTS
Licensure:
Current licensure in South Carolina required as a Licensed Independent Social Worker (LISW-CP preferred) or Licensed Clinical Social Worker (LCSW), Licensed Masters Social Worker (LMSW) that is in the process of obtaining LISW. A Master's Degree in Social Work from a program accredited by a nationally recognized accrediting body for social work programs is required. Minimum of two years post-masters Counseling experience related to assessment and treatment of substance abuse disorders. Initiative and stamina to work independently as well as with Ability to communicate effectively and clearly in written and oral form in English (ability to speak and read Spanish is a huge plus). Ability to work well under Ability to work with minimal Knowledge of communication resources and referral Ability to establish and maintain effective networking relationships with community Working knowledge of federal/state/local laws and regulations regarding eligibility for Must have a valid driver's license to allow travel to all locations as necessary and for business purposes.
