This is an outstanding opportunity for a Licensed Professional Counselor (LPC), Clinical Social Worker (LISW-CP or LMSW), Licensed Marriage and Family Therapist (LMFT), or associate-level clinician who is passionate about improving the lives of children, adolescents, adults, and families . Diagnose behavioral health conditions within your scope of practice. Develop individualized treatment plans using evidence-based practices. Provide individual, family, and crisis counseling. Deliver trauma-informed, culturally responsive behavioral healthcare. Provide behavioral health interventions for children, adolescents, adults, and families. Collaborate with primary care providers in an Integrated Behavioral Health model. Coordinate services with schools, social service agencies, hospitals, and community organizations. Maintain timely, accurate, and compliant electronic medical records (EMR). Participate in multidisciplinary case conferences and treatment planning. Connect patients with community resources and support services. Promote whole-person health through integrated medical and behavioral healthcare.
