Duties and Responsibilities:

Participate in the development and implementation of Individual Service Plan for individuals in treatment Plan and implement interventions that assist with helping individual reach goals of developed service plan Complete progress notes that meet the state and federal regualtions Meet with individual weekly in home and community to provide treatment Works closely with other agency staff and/or external providers for the purpose of service facilitation. Develops and maintains positive, collaborative relationships with community providers when interaction is necessary Participate in meetings for the purposes of supervision

Requirements:

Virginia Board of Counseling QMHP certification or ability to apply for cerification CPR and First Aide Training (to be obtained within 3 months of employment) Restraint Training (to be obtained within 3 months of employment)
