Position Highlights:
Join an integrated care team connecting behavioral and physical health services Provide brief, solution-focused behavioral health interventions Collaborate with primary care providers to support whole-person care Help patients navigate resources, referrals, and ongoing behavioral health needs Provide care management for patients with chronic behavioral health conditions Educate patients and staff on behavioral health prevention and treatment strategies Document patient care and interventions within an electronic medical record system Participate in program evaluation, training, and continuous improvement efforts Make a meaningful impact by expanding access to behavioral healthcare in the community
Qualifications:
Master s degree in social work, counseling, or related behavioral health field Licensed or license-eligible behavioral health professional (LMHC, LMHC-A, LICSW) Bilingual/biliterate in English and Spanish required Knowledge of behavioral health theory, case management, and ethical practices Experience providing behavioral health interventions and counseling services Valid driver s license
Compensation/Benefits:
