Job Description
As part of the primary care treatment team, the Behavioral Health Consultant (BHC) provides behavioral health assessment, diagnosis, treatment, and care coordination services. The BHC provides skill training through psychoeducation, patient education strategies and develops specific behavioral change plans for patients. The BHC delivers evidence-based therapeutic services, crisis intervention, and care coordination while maintaining compliance with FQHC, state, federal, and payer requirements.
