As part of the primary care treatment team, the Behavioral Health Consultant (BHC) provides behavioral health assessment, diagnosis, treatment, and care coordination services. The BHC provides skill training through psychoeducation, patient education strategies and develops specific behavioral change plans for patients. The BHC delivers evidence-based therapeutic services, crisis intervention, and care coordination while maintaining compliance with FQHC, state, federal, and payer requirements.

MINIMUM JOB REQUIREMENTS

Master's Degree in Social Work, Counseling, Marriage and Family Therapy, Psychology, or related behavioral health field from an accredited institution. Current Wisconsin licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Practical Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT). Maintain BLS certification.

Key Accountabilities:

Conduct comprehensive behavioral health assessments, psychosocial evaluations, and risk assessments for patients and/or students referred for services. Develop, implement, and monitor individualized treatment plans utilizing evidence-based therapeutic interventions. Collaborate with primary care providers, parents/guardians, community agencies and school staff as needed, to support treatment goals and continuity of care. 4. Provide appropriate referrals to other community and health and human services resources to supplement treatment. Maintain accurate, timely, and complete documentation within the electronic health record (EHR) in accordance with organizational, regulatory, and payer requirements. 6. Facilitate referrals to internal and external resources, including specialty behavioral health, psychiatric services, and community-based support programs. Promote mental health awareness and provide behavioral health education to students, families, and school personnel. Participate in quality improvement initiatives, supervision, training, and staff meetings to support program goals and professional development. 9. Perform other duties as assigned.

Pay:

$28.38 - $36.18 per hour
