Mission-driven organization focused on health equity and innovation Duties and Responsibilities Provide mental health and/or substance abuse counseling to individuals across the lifespan. Conduct assessments and evaluations to determine diagnosis using DSM criteria. Identifying treatment goals and develop holistic treatment plans. Implement evidenced-based treatment modalities to appropriately address diverse client needs. Continually assess client progress while tailoring approaches to individual needs. Collaborate with clients' family members, caretakers, and other service providers as necessary. Record and maintain records, medical documentation, and appointment schedules. Collect outcome data. Provide resources and referrals for client needs. Communicate with referring and partner agencies. Effectively use digital technology platforms (electronic medical records, telehealth, IP phone applications, etc) Minimum Qualifications Minimum of master's in Social Work, Professional Counseling, or similar degree from an accredited university program. Experience providing culturally sensitive mental health treatment and/or working with diverse populations. Excellent interpersonal communication skills. Proven relationship-building skills with individuals from diverse backgrounds. Experience working collaboratively with partnering organizations.

Wisconsin licensure as:
LCSW LPC LMFT
Preferred:
