GENERAL SUMMARY

Individual & Family Therapist work as part of the treatment team by providing individual, group and family therapy to youth in St. Charles care and their families or representatives as appropriate. They work closely with staff, treatment providers, state and county case managers or social workers, and court officials to prepare youth for a smooth transition to the community once care is complete. Assists with discharge planning in order to provide continuity of care to clients and family members.

ESSENTIAL FUNCTIONS

• Provides weekly individual, group and family therapy in-person or virtually as appropriate, with the primary focus on in-person when possible.
• Develops, modify, and monitors treatment plans.
• Identify and document behavioral goals for each youth and incorporate strategies to help youth accomplish said goals.
• Works in conjunction with systems partners to support clients/youth.
• Ensures timely completion of agency and program related documentation such as case notes, reports, etc.
• Participates in weekly individual and group supervision with the Director of Behavioral Health.
• Attends all relevant trainings, meetings, and staffings (times and days vary).
• Attend medication management appointments with youth in residential care.
• Support and assist the child and family treatment team with discharge planning. o To include community supports and appts being set up prior to discharge.
• Interpret children's code; reports state mandated issues regarding abuse.
• Adheres to St. Charles Youth & Family Services Policies and Procedures.
• Supports the goals of the agency mission.
• Other duties as assigned.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

• Master's degree in Social Work or related field; professional license preferred (APSW, LCSW, LPC, LPC-IT, PhD
• all considered)
• Minimum of 3 years direct experience working with children and families.
• Knowledge of crisis intervention, anger management and conflict resolution techniques.
• Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
• Ability to coordinate and conduct individual, family and group meetings.
• Attention to detail and good organizational skills.
• Proficiency in verbal and written communication.
• Proficiency in formal report writing regarding recommendations and summaries.
• Knowledge of State of Wisconsin child abuse/neglect.
• Knowledge of DSM equivalent version at time of hire as used within the field at the time.
• Knowledge and understanding of cross-cultural differences; with the ability to relate and communicate with persons from diverse cultural and socioeconomic backgrounds.
• Ability to facilitate team process and task completion, to cooperate with other family preservation teams and participate in multi-disciplinary teams for staffing the youth population on the units.
• Ability to manage physically aggressive clients. (Training provided)
• Valid Wisconsin driver's license and valid insurance required.
