Job Description

About the Job Family Centered Services of Alaska is seeking a Case Manager to oversee delivery of services and facilitate treatment to clients in our Foster Care Program. The Case Manager is responsible for maintaining the philosophy and effectiveness of the Therapeutic Family Group Homes program in Fairbanks, acting as a liaison between the client, family, clinicians, and community agencies, arranging for after-care services at client discharge, and collaborating with and providing support to FCSA clinicians. Some on-call work and schedule flexibility is required.
Responsibilities:
1. Facilitate family and inter-agency collaboration. 2. Advocate for the rights and needs of the youth and act as a community liaison who will represent FCSA to the professional and social community. 3. Prepare initial and 90-day client treatment plans. 4. Monitor preparedness for client discharge in collaboration with the FCSA clinician and treatment team. 5. Monitor behavior and progress in relation to the client's individual treatment plan. 6. Identify gaps in each youth's Plan of Care and take proactive steps to connect them with resources or supports, create appropriate services as directed by clinical staff and treatment team when a critical need cannot be met more traditionally, and coordinate any alternate treatment placement arrangements for the client, as needed. 7. Review and process all incident reports. 8. Contact parents/guardians regularly to discuss major issues/progress. 9. Collaborate and communicate with the FCSA clinician as needed 10. Coordinate on/and off-site visits, supervised phone calls. 11. Schedule treatment team meetings in accordance with Quality Assurance timelines and standards. 12. Create and update emergency binders at minimum weekly. 13. Maintain all FCSA paperwork in accordance with FCSA Quality Assurance standards and timelines. 14. Ensure all client documentation is entered into the EMR in accordance with FCSA Quality Assurance standards and timelines. 15. Adhere to billing requirements as established. 16. On-call rotation as scheduled. 17. Other duties as assigned.
Qualifications:
1. Bachelor's degree or equivalent work experience in psychology, social work, justice, guidance and counseling, sociology, education, nursing, or other human service field preferred. 2. High school diploma or GED, and two years working with adolescents in a mental health setting required. 3. Must be a minimum of 21 years of age. 4. Ability to work independently and make decisions with minimal supervision. 5. Ability to communicate in a professional manner with appropriate stakeholders (Treatment Team Members, outside agencies, program director). 6. Proficient office skills including time management, computer, Electronic Medical Records (EMR), strong verbal and written skills, and ability to complete required documentation in a timely/accurate manner. 7. Physical ability to employ Verbal and Physical aggression control techniques after being certified. (Ability to Physically Restrain adolescent clients). This may require you to lift up to 40 lbs., bend your knees, extend arms, and shoulders and other physical movements necessary to safely restrain a client. 8. Knowledge of State of Alaska childcare regulations and juvenile justice laws. 9. Freedom from previous criminal convictions that may adversely affect the employee's potential performance contingent on a fingerprint and/or records check. 10. Current Alaska driver's license, personal vehicle insurance, and driving history that allows for insurability by FCSA auto insurance carrier. 11. Current first aid and CPR certificate or ability to become certified. This position is a driving position . Applicants will be required to submit a driving record if selected for the position. All applicants must be at least 21 years of age due to State of AK regulations and must pass a State of AK Background Check. Today, FCSA has over 100 employees dedicated to our mission of providing unconditional care to children and their families. FCSA provides behavioral and mental health services to youth aged 5-19. Services include outpatient therapy, education, foster care, and short or long-term residential treatment. FCSA is nationally accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) and was one of the first organizations to be licensed by the state to establish, recruit, and monitor foster homes.

Pay:
$22.00 - $23.34 per hour Expected hours: 40.0 per week
