Job Description

Position Summary:
As a Behavioral Health Case Manager, you will provide care coordination services to individuals with behavioral health disorders, working with healthcare professionals and community organizations to develop and implement treatment plans, advocate for Participants' needs and rights, and provide education and support to Participants and their families. Primary Duties and Responsibilities (including but not limited to): The successful candidate will: Develop and implement individualized treatment plans in collaboration with healthcare professionals, Participants, and their families. Monitor and assess Participants' physical and emotional well-being and adjust treatment plans as needed. Coordinate referrals to appropriate healthcare professionals and community resources, such as housing or employment services. Advocate for Participants' needs and rights within the healthcare system and help Participants navigate complex healthcare systems. Provide education and support to Participants and their families to promote wellness and recovery. Conduct community outreach and relationship-building activities to increase awareness of available services and expand the Participant base, including visiting healthcare and community facilities to meet with providers and discuss available services. Attend community events, resource fairs, and other outreach opportunities to promote services and establish relationships with potential referral partners. Develop and distribute outreach and informational materials to promote services to healthcare providers, community organizations, Participants, and other potential referral sources. Maintain accurate and detailed records of Participant progress and treatment plans and ensure compliance with relevant laws and regulations. Collaborate with other healthcare professionals and community organizations to ensure continuity of care for Participants. Attend and participate in training and professional development opportunities to enhance knowledge and skills. Adhere to ethical and professional standards of conduct and confidentiality. Transport Participants via company vehicle for service provision as needed; adhere to Knik Tribe Transportation Policy. Work effectively in a multi-disciplinary team environment. Coordinate and maintain Participant scheduling. Perform other duties as assigned. Knowledge, Skills, And Abilities Knowledge of behavioral health principles, care coordination, person-centered services, and community-based resources. Strong communication and interpersonal skills, with the ability to build positive relationships with Participants, families, healthcare providers, referral sources, and community partners. Ability to assess Participant needs, coordinate services and referrals, provide education and advocacy, and assist individuals and families in navigating healthcare and community service systems. Ability to maintain accurate, timely, and confidential documentation while adhering to applicable laws, regulations, ethical standards, and professional boundaries. Ability to conduct community outreach, promote available services, develop relationships with potential referral sources, and represent Knik Tribe professionally in community settings. Strong organizational and problem-solving skills, with the ability to manage multiple Participants and priorities, work independently and as part of a multidisciplinary team, and adapt to changing needs and circumstances. Proficiency with Microsoft Office and computerized maintenance management or work order systems. Work Environment and Physical Requirements Sitting or standing for extended periods of time: May need to sit or stand for extended periods of time while conducting assessments, providing therapy sessions, or completing paperwork.
Lifting and carrying:
May need to lift and carry equipment, files, or other materials as part of their job responsibilities.
Manual dexterity:
May need to use their hands to type, write, or manipulate small objects, such as therapy materials or instruments.
Visual acuity:
May need to have good vision to read Participant files, complete paperwork, and observe Participant behaviors.
Hearing:
May need to have good hearing to listen to Participants, colleagues, and other professionals.
Mobility:
May need to move around the clinic to attend meetings, collaborate with colleagues, or provide services in different locations.
Minimum Requirements:
Associates degree in Human Services, Counseling, Behavioral Health, or related field; Or an equivalent combination of education and experience. Two (2) years of experience working in a behavioral health setting. Three (3) years of administrative experience. Valid Alaska driver's license with a satisfactory driving record required. Clean state and federal background check.

Salary:
$24.00 - $33.00 per hour DOE/E.
