The position requires the Case Manager to be the main link and coordinate care between the Veteran and the VA hospital, medical providers, community agencies, both public and private, and perform outreach. Preferred Master's Degree in the Behavioral Sciences and minimum of 3 years direct experience working with Veterans who are in a crisis situation. The CM will conduct comprehensive intake, develop an individualized care and exit plans. The salary range is $63,523—$77,212 plus benefits.
