In this vital role, you will support social workers and healthcare professionals in delivering comprehensive patient-centered care across various settings, including hospitals, outpatient clinics, hospice, and community-based programs. Your energetic approach and attention to detail will help ensure seamless coordination of services, accurate documentation, and meaningful support for patients and their families. Responsibilities Assist with patient intake, assessments, and data collection to support clinical evaluations and treatment planning Support social workers with documentation of medical records, progress notes, and compliance with HIPAA and clinical ethical guidelines Participate in crisis intervention and behavioral health support, including behavioral therapy and motivational interviewing techniques Contribute to group therapy sessions, individual counseling, and psychosocial assessments under supervision Help with hospice and end-of-life care activities, including supporting families through grief counseling and bereavement processes Skills Strong knowledge of social work practice standards, mental health counseling, and clinical counseling techniques Familiarity with CBAS center Excellent communication skills for patient assessment, group therapy facilitation, crisis management, and community resource coordination Knowledge of healthcare regulations such as HIPAA and clinical ethical guidelines; basic life support (BLS) certifications preferred Join us in delivering compassionate care that transforms lives!
