Mental Health Case Manager - LADMH / Interim HousingJob Summary We are seeking a compassionate, dependable, and dedicated Mental Health Case Manager to join our team supporting individuals residing in interim housing and participating in Los Angeles Department of Mental Health (LADMH) services. The Mental Health Case Manager will provide client-centered case management, behavioral health support, care coordination, and connections to community resources for individuals experiencing homelessness, mental health challenges, substance use, and other complex social and behavioral health needs. This position plays an important role in helping clients achieve greater stability by connecting them to mental health services, healthcare, housing opportunities, benefits, and other supportive resources. The Case Manager will work closely with clients, behavioral health providers, housing staff, healthcare professionals, and community partners to develop individualized service plans and support clients throughout their transition from interim housing toward permanent housing and greater independence. The ideal candidate is compassionate, trauma-informed, organized, and comfortable working with individuals who may have complex behavioral health, medical, housing, and social needs. Essential Duties and Responsibilities Conduct comprehensive assessments of clients' mental health, social, housing, medical, and supportive service needs. Develop and maintain individualized service plans in collaboration with clients and the appropriate care team. Provide ongoing case management and supportive services to clients residing in interim housing. Support clients in accessing and engaging with LADMH services and other behavioral health resources. Coordinate referrals and connections to mental health providers, substance use treatment programs, primary care, crisis services, and other community-based resources. Assist clients with navigating the behavioral health system and advocating for appropriate services. Monitor client engagement and progress toward established goals and update service plans as needed. Coordinate with LADMH providers, healthcare providers, housing staff, social workers, and other community partners to ensure continuity of care. Assist clients with obtaining identification, Medi-Cal, Social Security benefits, disability benefits, food assistance, and other eligible public benefits. Support clients in completing applications and documentation necessary to access housing, healthcare, and supportive services. Assist with housing navigation and housing retention by helping clients prepare for and transition from interim housing to permanent housing. Provide referrals and warm handoffs to appropriate mental health, substance use, healthcare, housing, and community resources. Assist clients in developing independent living skills and strategies that promote stability and successful housing placement. Provide crisis support and assist in connecting clients to appropriate crisis intervention or emergency services when necessary, following organizational protocols. Maintain regular contact with assigned clients and actively engage individuals who may be difficult to reach or hesitant to participate in services. Utilize trauma-informed, culturally responsive, and person-centered approaches when working with clients. Maintain accurate, timely, and thorough case notes, assessments, service plans, referrals, and other required documentation. Complete all required LADMH and program documentation in accordance with established guidelines and timelines. Maintain confidentiality and comply with HIPAA, LADMH, organizational policies, and applicable regulations. Participate in case conferences, team meetings, trainings, staff meetings, and other program activities. Establish and maintain professional relationships with community organizations, behavioral health providers, housing programs, and other service partners. Advocate for clients while maintaining appropriate professional boundaries. Perform other duties as assigned. Qualifications and Requirements Bachelor's degree in Social Work, Psychology, Behavioral Health, Human Services, or a related field preferred. Relevant experience may be considered in lieu of a degree. Previous experience in mental health, behavioral health, case management, homelessness services, interim housing, supportive housing, or social services preferred. Experience working with individuals experiencing homelessness, mental health conditions, substance use disorders, or co-occurring behavioral health and social needs is highly desirable. Familiarity with LADMH services, behavioral health systems, Coordinated Entry, interim housing, permanent supportive housing, or other homeless services programs is preferred. Knowledge of Los Angeles-area community resources and behavioral health programs is a plus. Strong understanding of trauma-informed care, client advocacy, and person-centered case management. Strong organizational and time-management skills with the ability to manage multiple client cases and competing priorities. Excellent written and verbal communication skills. Ability to build trusting relationships with clients while maintaining appropriate professional boundaries. Ability to work collaboratively with multidisciplinary teams and external service providers. Proficiency with Microsoft Office and electronic case management/documentation systems. Ability to maintain accurate, timely, and confidential client records. Must demonstrate professionalism, reliability, compassion, and a commitment to serving individuals experiencing homelessness and behavioral health challenges. Bilingual or multilingual skills are a plus. Preferred Experience and Skills Experience working directly with LADMH or other county behavioral health programs. Experience working in interim housing, permanent supportive housing, recuperative care, shelter, or homeless services. Familiarity with HMIS or other homeless services databases. Knowledge of Medi-Cal, CalFresh, General Relief, SSI/SSDI, housing programs, and other public benefits. Experience assisting clients with housing applications, housing documentation, and housing transitions. Experience working with substance use disorders and co-occurring mental health conditions. Familiarity with crisis intervention, de-escalation, motivational interviewing, and trauma-informed practices. Experience coordinating care between behavioral health providers, healthcare providers, housing programs, and community organizations. Work Environment This position is primarily community-based and may be performed within interim housing facilities, office settings, client residences, healthcare facilities, and other community locations. The Case Manager may work with clients experiencing complex behavioral health, medical, substance use, and housing-related challenges. The position requires the ability to work respectfully and effectively with individuals from diverse backgrounds and to maintain professionalism in situations that may involve crisis, conflict, or challenging behaviors. $26.00 - $28.00 per hour
