DEFINITION / JOB SUMMARY

The Clinician is a California-licensed LMFT or LCSW who provides behavioral health clinical services, consultation, and clinical support within Habitat for Humanity Yuba/Sutter's integrated housing and supportive services continuum. A primary focus of the position is the Family-Centered Behavioral Health Housing Stabilization Network (FBH-HSN) Innovation Project, which embeds early behavioral health identification, engagement, stabilization, and navigation within housing and community settings. The Clinician supports a person-centered, family-centered, trauma-informed, culturally responsive, harm-reduction-oriented approach designed to identify needs early, reduce barriers to care, and strengthen linkages to appropriate behavioral health, health, housing, and social services. As Habitat's behavioral health continuum expands, the Clinician may also provide clinical services and support to Outpatient and Residential Substance Use Disorder (SUD) treatment programs and other behavioral health programs. The position works collaboratively with behavioral health leadership, Community Health Workers (CHWs), Peer Support Specialists, SUD counselors, care coordination staff, housing staff, county behavioral health partners, Medi-Cal Managed Care Plans, and community providers.

HOURS & LOCATION

Full-time position, generally 40 hours per week. Work may occur at Habitat program sites, participant homes, community locations, outpatient and residential treatment settings, and other service locations in Yuba, Sutter, and surrounding counties. Some evening, weekend, or on-call availability may be required based on program needs.

SUPERVISION RECEIVED AND EXERCISED

• Reports to designated Behavioral Health leadership or other executive/department leadership as assigned.
• May provide functional, clinical, or day-to-day direction to subordinate or assigned staff, including CHWs, Peer Support Specialists, SUD counselors, interns, and other behavioral health/support staff, within the scope of the Clinician's role and licensure.
• May assist management with case assignment, workflow direction, case consultation, chart review, performance feedback, and monitoring adherence to clinical and program standards.
• Formal personnel actions and administrative supervision remain with the designated manager unless specifically delegated by Habitat.

ESSENTIAL FUNCTIONS

• Provide behavioral health assessment, clinical screening, brief intervention, individual and family-focused clinical services, crisis assessment, stabilization support, care planning, and referral/navigation consistent with licensure, program design, and participant needs.
• Support implementation of the FBH-HSN Innovation Project by helping develop and refine behavioral health screening protocols, clinical workflows, escalation criteria, referral pathways, documentation standards, and approaches for warm linkage to county, Medi-Cal, and community behavioral health services.
• Provide clinical consultation and high-acuity case guidance to CHWs, Peer Support Specialists, care coordinators, housing staff, and other team members while maintaining appropriate professional scope and boundaries.
• Participate in interdisciplinary case conferences and coordinate with internal and external providers to support continuity of care and reduce fragmentation across housing, behavioral health, physical health, SUD, and social service systems.
• Assess safety and clinical risk and respond appropriately to behavioral health crises, including coordination with crisis services, emergency services, county behavioral health, and other providers when a higher level of care is indicated.
• Maintain culturally responsive, trauma-informed, person-centered, family-centered, recovery-oriented, and harm-reduction practices when serving low-income, rural, unhoused or housing-insecure, justice-involved, and other underserved populations.
• Provide clinical documentation that is timely, accurate, complete, and compliant with applicable program, payer, contractual, confidentiality, and regulatory requirements.
• Participate in clinical chart review, quality assurance, utilization review, and continuous quality improvement activities; identify documentation or service-delivery concerns and provide corrective guidance within assigned authority.
• Support collection and interpretation of program and outcome data, including information needed for Innovation Project quality improvement, reporting, and evaluation activities, while protecting participant confidentiality.
• Assist in developing, reviewing, and implementing behavioral health policies, procedures, protocols, forms, training materials, and practice standards.
• Provide training, coaching, and educational presentations to Habitat staff on behavioral health topics such as trauma-informed care, motivational interviewing, de-escalation, suicide and crisis awareness, professional boundaries, co-occurring disorders, SUD, harm reduction, engagement strategies, and other topics within the Clinician's competence.
• Represent Habitat, as assigned, in meetings with county behavioral health, Managed Care Plans, treatment providers, community partners, funders, and other stakeholders.
• Participate in staff meetings, case conferences, quality improvement meetings, program implementation meetings, required trainings, and professional development activities.
• Maintain professional competence, ethical practice, and California licensure in good standing; obtain continuing education required for licensure, program participation, contracts, or assigned clinical responsibilities.

SUD OUTPATIENT AND RESIDENTIAL TREATMENT SUPPORT

As Habitat develops and expands its SUD treatment continuum, the Clinician may be assigned responsibilities supporting Outpatient and Residential SUD programs consistent with the Clinician's license, competence, program certification, payer requirements, and applicable law.

Duties may include:

• Provide clinical expertise and consultation to SUD counselors and interdisciplinary treatment teams serving participants with SUD and co-occurring mental health conditions.
• Review, provide feedback on, and when authorized approve clinical assessments, treatment plans, level-of-care documentation, and other clinical records consistent with applicable ASAM, Drug Medi-Cal Organized Delivery System (DMC-ODS), CalAIM, county, and program requirements.
• Support determination and documentation of medical necessity or other clinical eligibility requirements when within the Clinician's authorized scope.
• Provide individual, family, and group clinical services and temporary clinical coverage as needed, including services for participants with co-occurring mental health and substance use conditions.
• Support clinical coordination across outpatient, residential, withdrawal management, medication treatment/MAT, mental health, medical, recovery support, housing, and community-based services.
• Assist with clinical quality assurance, chart review, corrective action, program monitoring, certification/licensure readiness, and implementation of new or revised treatment standards.

KNOWLEDGE OF

• Clinical assessment, diagnosis, treatment planning, crisis intervention, evidence-informed psychotherapy, and behavioral health care coordination.
• Serious mental illness, substance use disorders, co-occurring conditions, trauma, homelessness/housing instability, and justice-involved populations.
• Family systems and the impact of behavioral health conditions on household functioning, housing stability, and caregiver/family relationships.
• Trauma-informed care, motivational interviewing, harm reduction, recovery-oriented services, cultural humility, and engagement of individuals who may be ambivalent about treatment.

HIPAA, 42 CFR

Part 2 when applicable, California confidentiality requirements, mandated reporting, professional ethics, and documentation standards.

• ASAM Criteria and California SUD treatment systems, including DMC-ODS and CalAIM, preferred.
• Quality improvement, clinical documentation review, program compliance, and outcome-focused service delivery.

REQUIRED SKILLS / ABILITIES

• Strong clinical judgment and ability to assess risk, prioritize needs, and determine appropriate intervention or referral.
• Ability to work effectively in nontraditional settings, including housing sites, participant homes, community locations, and SUD treatment environments.
• Ability to provide clinical leadership and direction without unnecessarily medicalizing community-based, peer, or CHW services.
• Ability to train and coach multidisciplinary staff with varying levels of behavioral health experience.
• Excellent interpersonal, verbal, written, facilitation, and documentation skills.
• Ability to establish effective working relationships with participants, families, staff, public agencies, healthcare partners, and community organizations.
• Ability to organize competing priorities, maintain confidentiality, exercise sound judgment, and work both independently and collaboratively.
• Proficiency with electronic health records, data systems, Microsoft 365 or comparable business software, and telehealth technology as applicable.

EDUCATION, LICENSURE, AND EXPERIENCE

• Master's degree in Marriage and Family Therapy, Social Work, Counseling, or a closely related behavioral health field from an accredited institution.
• Current, unrestricted California license in good standing as a Licensed Marriage and Family Therapist (LMFT) or Licensed Clinical Social Worker (LCSW) is required.
• Clinical experience serving individuals and families with mental health, SUD, trauma, housing instability, or other complex psychosocial needs is required; experience in community-based or integrated care settings is strongly preferred.
• Experience with SUD treatment, co-occurring disorders, ASAM Criteria, DMC-ODS, CalAIM, Medi-Cal documentation, or residential/outpatient treatment is preferred.
• Experience providing staff training, clinical consultation, chart review, or direction of multidisciplinary staff is preferred.
• Bilingual/bicultural skills are preferred based on community and program needs.

PHYSICAL AND OTHER REQUIREMENTS

• Ability to perform work at a computer as well as stand, walk, and travel between program, housing, and community locations.
• Ability to traverse varied properties and environments, including locations with stairs and outdoor areas, with or without reasonable accommodation.
• Ability to lift and carry up to 20 pounds at times.
• Possess and maintain a valid California driver's license and meet Habitat's automobile insurance requirements when driving is an essential function of the assignment.
• Successfully complete background screening and other clearances required by Habitat, funders, contracts, licensing/certification bodies, or program partners.
• Maintain all professional licenses, certifications, training, and clearances required for assigned duties.
