Job Overview Provides psychosocial assessments and psychological interventions to PACE participants (when appropriate, including their family members), often presenting with complex psychosocial needs. Conduct consultations with care teams about behavioral health issues and situations faced by participants. Coordinates service delivery and assessment/treatment recommendations with the PACE multi-disciplinary team and, if appropriate, community agencies, to ensure appropriate participant care as outlined on the care plan. In some instances, we will provide clinical supervision to license-seeking MSW staff. Minimum Requirements Licensed Clinical Social Worker by the California Board of Behavioral Science. Minimum of 2 years of experience working with the elderly or disabled adults and/or mental health/substance abuse adult populations. Master's in social work required. A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
