Role Overview The Residential Case Manager provides integrated case management and support services within a co-occurring residential treatment setting serving adults with substance use and mental health disorders. This role supports residents throughout their treatment episode by coordinating care, facilitating engagement in treatment, assisting with discharge planning, and helping residents develop the skills necessary for sustained recovery and successful community reintegration. Residential Case Managers work collaboratively with interdisciplinary treatment, medical, psychiatric, nursing, and recovery support teams to address residents' behavioral health, medical, housing, legal, vocational, and social service needs. This position provides individual and group interventions, crisis support, service coordination, advocacy, and skill-building in a structured 24/7 therapeutic environment. Case Managers assist residents in navigating systems of care while promoting recovery, accountability, stability, wellness, and independent living skills.
Essential Duties:
Coordinate integrated case management services for residents receiving ASAM 3.5 and 3.7 residential treatment services for co-occurring substance use and mental health disorders. Collaborate closely with residential treatment staff, nursing, psychiatric providers, peer specialists, and external community partners to support continuity of care and individualized treatment planning. Facilitate resident engagement in treatment, recovery planning, and discharge preparation, including coordination of aftercare appointments, housing, transportation, benefits, and community-based services. Provide support, psychoeducation, and coaching related to emotional regulation, recovery maintenance, coping skills, interpersonal effectiveness, and activities of daily living. Support residents in developing independent living skills including medication management support, financial management, appointment coordination, personal organization, and community resource navigation. Complete needs assessments, service planning, progress documentation, discharge planning, and other clinical documentation in accordance with agency, Medicaid, ASAM, and regulatory standards. Assist residents in navigating complex systems including behavioral health, medical care, social services, criminal justice, probation/parole, housing programs, and benefits systems. Respond to and support behavioral health crises within the residential milieu using trauma-informed, recovery-oriented, and de-escalation approaches. Maintain therapeutic and professional boundaries while fostering a safe, structured, and recovery-oriented residential environment. Advocate for residents' needs while encouraging personal responsibility, self-efficacy, and long-term recovery goals. Develop and maintain collaborative relationships with community providers, referral sources, housing agencies, and recovery support organizations to enhance continuity of care and successful resident transitions.
Minimum Qualifications:
Bachelor's degree in behavioral science required At least one year of experience in directly relevant experience in the Mental Health and/or Substance Use Disorder Field required
Licenses and Certifications:
Valid Colorado driver's license or ability to obtain within 30 days of hire and a clean driving record CAS and/or ability to obtain CAS credential preferred
