Responsibilities Conduct thorough patient assessments to identify social needs, emotional well-being, and support systems Provide specialized counseling services including child & family counseling, behavioral therapy, mental health counseling, and addiction counseling Coordinate community resources such as housing, financial assistance, and support groups to enhance patient care outcomes Assist with discharge planning by collaborating with medical teams to ensure seamless transitions from hospital or facility to home or assisted living Document all client interactions accurately within medical records following HIPAA guidelines and social work practice standards Support crisis intervention efforts and manage urgent cases involving PTSD, dementia, Alzheimer's care, or developmental disabilities Engage in clinical research activities related to social work practice improvements and patient care strategies Skills Proven experience in social work practice standards with a focus on medical social work and behavioral health Strong knowledge of medical terminology and Medicare policies for accurate documentation and billing Expertise in clinical counseling techniques such as motivational interviewing, cognitive behavioral therapy, individual/group therapy, and crisis management Ability to work effectively with individuals from diverse cultural backgrounds and across age groups including geriatrics and pediatrics Familiarity with hospice & palliative medicine principles, end-of-life care practices, and assisted living environments Excellent communication skills for client interaction, interdisciplinary collaboration, and case management Competence in handling medical records securely while adhering to HIPAA regulations and ethical guidelines Join us in delivering compassionate support that transforms lives!
