Job Overview We are seeking a compassionate and highly skilled Certified Behavior Health Case Manager to join our team. In this vital role, you will coordinate and deliver behavioral health services to children and adolescents within our community, ensuring their mental health needs are met with professionalism and empathy. Your expertise will support families, facilitate discharge planning, and promote positive outcomes through comprehensive case management and community resource coordination. Duties Conduct thorough diagnostic evaluations and assessments to identify behavioral health needs of children and adolescents. Develop individualized treatment plans that incorporate clinical case management, behavioral health interventions, and community resource linkage. Collaborate closely with healthcare providers, social workers, families, and educational teams to ensure coordinated care delivery. Manage electronic health records (EHR) accurately while adhering to HIPAA regulations to protect patient confidentiality. Provide mental health counseling, addiction counseling, or social work support tailored to clients' unique circumstances. Facilitate discharge planning by coordinating post-acute care services, outpatient therapy, or community-based supports. Prioritize tasks effectively in a fast-paced environment while maintaining high-quality patient service and patient care standards. Maintain up-to-date knowledge of medical terminology, physiology, and trauma-informed practices relevant to hospital medicine, ICU, emergency medicine, or pediatric care settings. Qualifications Valid certification as a Behavior Health Case Manager or equivalent credential. Proven experience working in this field Strong background in behavioral health with experience in inpatient or outpatient environments involving children or individuals with disabilities. Knowledge of medical records management systems (EMR/EHR), HIPAA compliance, and medical terminology. Experience working with individuals with substance use disorders, mental health conditions, or complex trauma histories. Familiarity with managed care processes, utilization management, and discharge planning procedures. Excellent client communication skills along with the ability to work effectively with families from diverse cultural backgrounds. Ability to prioritize tasks efficiently while demonstrating strong organizational skills in high-pressure situations.
