Role:
In this leadership role, you'll oversee a community-based care model that integrates behavioral health, medical services, and social supports. You'll guide your team in delivering person-centered care, building strong relationships with clients and families, and ensuring individuals receive the comprehensive support they need to thrive. You'll also play a key role in driving quality outcomes, supporting care coordination, and helping individuals successfully manage their health while reducing barriers to care. As a FACT Manager you will:
  • Conduct outreach activities to promote home Health and engage insurance clients to opt in to FACT-lite team model service.
  • Complete assessments and screenings with members to provide feedback and insight into how to improve behavioral health and medical related behaviors (under consult from nurse, physician, APRN assessing the total needs of the client).
  • Develop Plan of Care to include linkage for the client and family with services and identified resources.
  • Links client to primary care, behavioral health, health specialty services and community/peer social supports and other natural supports.
  • Organize and facilitate access to, and monitoring of progress toward goals in the Plan of care to manage chronic mental health or co-occurring conditions for health and wellness though face to face and collateral contacts with the member, family, caregivers, physical care, and other providers.
  • Travel with team on community visits and perform clinical services to FACT clients.
  • Carry a reduced caseload, performing clinical duties.
  • Work closely with FACT administrative assistant to gather data, submit required monthly FACT reports and oversee discretionary funds supporting client's independence Who are we looking for?
  • Master's degree from an accredited university with major course work in the areas of health related/psychology/social work areas.
  • Three years' experience in Mental health managerial positions strongly preferred.
  • Five years working with individuals dealing with Severe and Persistent Mental Illness and or co-occurring mental health and substance use disorders. Active LCSW, LMFT or LMHC license in State of Florida require. Must possess a valid Florida driver's license and have acceptable driving record. Why You'll Love Working Here
