Division/Department/Program:

Rockdale Outpatient Center/CCBHC A new service model, Certified Community Behavioral Health Clinics (CCBHC), offers comprehensive, coordinated, and integrated care to individuals living with mental health and substance abuse treatment needs. CCBHCs provide outreach, increase access, improve services, and serve as a "one-stop-shop" to those who are currently underserved with mental health and substance use disorders. The CCBHC Licensed Clinician (Bilingual) provides professional therapeutic services using identified Evidenced Based Practices to persons with mental illness, behavior challenges, and/or substance use disorders.

Duties & Responsibilities:

Provides individual, family or group therapeutic counseling and/or educational and skilldevelopment sessions as indicated in the service plan.

Monitors client progress and compliance with the service plan.

Writes timely progress notes that relate directly to the treatment outcomes described inthe service plans.

Assembles, analyzes, and interprets new/updated information about the client toreassess treatment needs.

Identifies unmet needs, reviews available resources, and modifies the treatment plan asindicated.

Facilitate communication, linkage, and continuity of care between clients, physicians, other organizational components, community resources and service providers.

Establishes linkages and coordination of services with community agencies.

Develops and maintains cooperative working relationships with community service providers; participates in treatment team meetings.

Informs clients and families of the availability of community advocacy and support groups.

Provides crisis intervention services to existing and walk-in clients as needed.

Provides on-call, after-hours services on a rotational schedule.

Other program related duties as assigned.

Minimum Qualifications:

Master's degree in social work and counseling from an accredited college or university and eligibility to be licensed and two (2) years of supervised experience in the area of assignment or two (2) years of experience at the lower-level social worker 1 (HCP050) or position equivalent.

Note:

Position requires licensure in the state of Georgia as an

LMSW, LAPC, LCSW, or LPC Preferred Qualifications:

Ability to read, write, and speak fluently in English and Spanish.

Requirements/Competencies:

Must be able to lift 20 pounds. Requires long periods of sitting/standing. Must have valid Georgia drivers license and Motor Vehicle Record in accordance with company policy. Candidates for selection must pass a criminal background check (including fingerprinting). Must pass a pre-employment drug screen and subject to random drug screens.
